Prisma Health is conducting a Community Health Needs Assessment (CHNA) throughout its service areas in South Carolina.
As part of its commitment to hear from its community members, the health care system completes a CHNA every three years.
Community Health Needs Assessment surveys are designed to identify top health care priorities in the community. Survey participation is anonymous. Collected information will not be attributed to any specific source. Once all data is compiled, Prisma Health will share the results of the completed report on PrismaHealth.org/CHNA. Previous reports and information about the CHNA also are provided on this website.
Prisma Health’s assessment process also includes conducting interviews with state and local elected officials, major employers, community members and community organizations that provide health services. In addition to online surveys, surveys links and forms are distributed through prepaid mailings, door-to-door, and at local organizations/agencies and community events.
